Immigration Physicals
Trust Medical Services offers immigration medical exam physicals. These immigration medical exams are required for individuals applying for citizenship, seeking Permanent Residency, or applying for a Green Card. These applicants must have an immigration physical before they can be approved. Dr. Hagan is a designated civil surgeon for the U.S. Department of Homeland Security and the U.S. Citizenship and Immigration (USCIS). After you have all the necessary tests and evaluations, you will be given an official, sealed document to provide to an immigration officer.
Required Documents:
- Alien number(A)
- A notice of receipt from USCIS, commonly know as I-797 for their I-130, I-140 or I-485
- Employment authorization
- Travel document
- A valid picture ID which could be a driver’s license, government or military issued ID or a passport
- Immunization record
Immigration Physical Package includes any/all of the following:
- T.B Skin Test (PPD Test)
- Tetanus vaccine (dTap)
- Syphilis Test (RPR Test)
- Measles, Mumps, Rubella immunity (need for anyone born after 1956 due to no vaccination record)
For most applications, the above vaccinations are usually sufficient to complete the exam; but in certain cases, additional adult vaccinations may be required, Depending on other health conditions, treatment or referral, further testing might be required. This will be determined by the doctor at the time of visit and after test results are back.
Additional Vaccinations that will be required due to the season and age of the applicant are:
- Pneumococcal Vaccine (required if 65 years or older)
- Influenza Vaccine (required in the winter season)
- Hepatitis B (required if no previous record for ages 19 or less)
